Parent epic: [E22] Upgrade safety (#50)

Intent

Explicitly document the operator response to a failed upgrade so recovery steps are clear.

Acceptance criteria

  • Operator response to a failed upgrade is explicitly documented
  • The documentation covers recovery from a failed migration

Explicitly out of scope

  • Recording the release manifest fields (E22-S01)
  • Running the pre-upgrade compatibility check (E22-S02)

Test plan

  • Review the documentation against a simulated failed upgrade and confirm steps are actionable

Rollback note

  • Documentation-only change; revert the doc content to roll back
