Parent story: [E29-S01] Upload endpoint and validation (#112)

Intent

De-duplicate uploads by checksum so identical bytes reuse the existing media record.

Acceptance criteria

  • Compute SHA-256 on ingest for every accepted upload
  • Return the existing media record when the checksum matches a stored duplicate instead of storing again

Explicitly out of scope

  • Size and MIME rejection is covered by E29-S01-T01
  • Safe object key generation is covered by E29-S01-T03

Test plan

  • Upload identical bytes twice and assert one record with the matching checksum is returned

Rollback note

  • Revert the de-duplication lookup; existing records are unchanged
