Parent epic: [E26] Failure-mode verification (#54)

Intent

Verify that PostgreSQL unavailability degrades safely: the dynamic application does not report ready (section 65).

Acceptance criteria

When PostgreSQL is unavailable the dynamic application does not become ready.
A clear diagnostic identifies the database unavailability.
No half-initialised page is served during the outage.

Explicitly out of scope

Core migration failure (E26-T02) and extension migration failure (E26-T03).
Other E26 failure modes.

Test plan

Stop the database and assert the application fails readiness with a clear diagnostic.

Rollback note

Test-only; restarting the database restores readiness.
