Intent

Verify that a duplicate extension ID degrades safely with a deterministic startup/activation error and clear diagnostic (section 65).

Acceptance criteria

A duplicate extension ID produces a deterministic startup/activation error.
A clear diagnostic identifies the duplicate ID and package.

Explicitly out of scope

PostgreSQL unavailable (E26-T01) and invalid settings (E26-T05).

Test plan

Register two extensions with the same ID and assert a deterministic error with diagnostic.

Rollback note

Test-only; removing the duplicate restores activation.
