Parent epic: [E13] Extension migrations (#41)

Intent

Register extension migrations in a ledger that records the extension ID, an immutable migration ID/version, and the applied timestamp.

Acceptance criteria

Each extension migration is recorded with its owning extension ID.
The migration ID/version is immutable once applied.
An applied timestamp is recorded for each applied migration.
Extensions migrate independently of core migrations through the same ledger.

Explicitly out of scope

The migration lock and transaction coordinator (E13-S02).
The migration error state (E13-S03).

Test plan

Apply an extension migration and assert the ledger row carries the extension ID, immutable ID/version, and a timestamp.

Rollback note

Remove the ledger rows and reverse the migration; follow the core migration recovery path.
