Intent

Register extension migrations in the migration ledger.

Acceptance criteria

Records the extension ID, an immutable migration ID/version, and an applied timestamp per migration.

Explicitly out of scope

The migration lock/transaction (E13-S02) and error state (E13-S03).

Test plan

Apply a migration and assert the ledger row has the expected fields.

Rollback note

Remove ledger rows and reverse the migration via the core recovery path.
