Parent epic: [E13] Extension migrations (#41)

Intent

Run extension migrations under the existing advisory-lock migration coordinator within transactions so concurrent runners are excluded and failures are atomic.

Acceptance criteria

Extension migrations run under the existing advisory migration coordinator.
The advisory lock prevents concurrent migration runners from applying migrations simultaneously.
Migrations run within a transaction so a failure leaves no partial schema.

Explicitly out of scope

Ledger registration of extension migrations (E13-S01).
The migration error state (E13-S03).

Test plan

Attempt two concurrent migration runners and assert the advisory lock serialises them.
Force a failing migration and assert the transaction rolls back cleanly.

Rollback note

Release the advisory lock and roll back the transaction; no manual cleanup beyond the ledger.
