Intent

Run extension migrations under the existing advisory migration coordinator within a transaction.

Acceptance criteria

Extension migrations run under the existing advisory migration coordinator.
Concurrent runners are excluded by the advisory lock and failures roll back transactionally.

Explicitly out of scope

Ledger registration (E13-S01) and error state (E13-S03).

Test plan

Run two concurrent runners and force a failing migration to assert serialisation and rollback.

Rollback note

Release the advisory lock and roll back the transaction.
