Parent epic: [E13] Extension migrations (#41)

Intent

Place a failed optional extension into an error state while keeping unrelated core routes available where possible.

Acceptance criteria

A failed optional extension transitions to the error lifecycle state.
Unrelated core routes remain available where possible.
Diagnostics are visible in admin and in structured logs.

Explicitly out of scope

Ledger registration of migrations (E13-S01).
The migration lock and transaction coordinator (E13-S02).

Test plan

Force an extension migration to fail and assert the extension is error while public/core routes still respond.

Rollback note

Clear the error state and re-run the migration after the fix; no content data is lost.
