Parent story: [E13-S03] Error state (#102)

Intent

Put a failed optional extension into an error state while keeping unrelated core routes available.

Acceptance criteria

A failed optional extension transitions to the error state.
Unrelated core routes remain available where possible with diagnostics in admin and logs.

Explicitly out of scope

Ledger registration (E13-S01) and the migration lock/transaction (E13-S02).

Test plan

Force an extension migration failure and assert error state with core routes still responding.

Rollback note

Clear the error state and re-run the migration after the fix.
