Parent epic: [E19] First-run setup (#47)

Intent

Detect when the installation has not completed first-run bootstrap, so the system makes no normal public authoring assumptions until bootstrap is complete.

Acceptance criteria

Uninitialised installations are detected before public authoring assumptions are made
The system withholds normal authoring behaviour while bootstrap is incomplete

Explicitly out of scope

Creating the administrator account (E19-S02)
The initial site wizard that collects required product data (E19-S03)

Test plan

Start a fresh stack with an empty database and confirm the uninitialised state is detected

Rollback note

Detection is read-only; no data or schema changes require rollback
