Parent epic: [E19] First-run setup (#47)

Intent

Create the administrator account during first-run setup without relying on any default credentials.

Acceptance criteria

The administrator account can be created as part of first-run setup
No default credentials exist for the administrator account

Explicitly out of scope

Detecting the uninitialised installation state (E19-S01)
Collecting site name and canonical URL (E19-S03)

Test plan

Run first-run setup and confirm an administrator is created with a supplied credential

Rollback note

Delete the created administrator row to return to the pre-setup state
