Intent

Enforce globally unique extension IDs across all loaded manifests.

Acceptance criteria

Extension IDs are enforced as globally unique.
Two manifests sharing an ID are detected.

Explicitly out of scope

Deterministic duplicate-ID error output (E02-S01-T05).

Test plan

A test loads colliding IDs and asserts detection.

Rollback note

Read-only check; no state is mutated.
