Intent

Emit a deterministic startup/activation error when a duplicate extension ID is found.

Acceptance criteria

A duplicate ID yields a deterministic startup or activation error.
The duplicate-ID error is reproducible across runs.

Explicitly out of scope

Package and constraint diagnostic content (E02-S01-T06).

Test plan

A test asserts the duplicate-ID error is deterministic.

Rollback note

The error path aborts activation cleanly; no state is persisted.
