Intent

Enforce revision numbers unique per content item.

Acceptance criteria

  • Revision numbers are unique per content item.
  • Duplicate revision numbers for one item are prevented.

Explicitly out of scope

  • Current-revision pointer (E04-S03-T03).

Test plan

  • A test asserts per-item revision-number uniqueness.

Rollback note

  • Unique constraint; revert drops the constraint.
