Intent

Prevent orphaned or inconsistent current revisions via a transaction.

Acceptance criteria

Revision insert and pointer update are atomic in one transaction.
No orphaned or inconsistent current revision can occur.

Explicitly out of scope

Admin readability of prior revisions (E04-S03-T05).

Test plan

A failure-injection test asserts no orphaned pointer remains.

Rollback note

Transaction rollback leaves no partial state.
