Intent

Persist extension runtime state fields so they survive restarts.

Acceptance criteria

Stores installed version, enabled state, config schema version, error diagnostics, and timestamps.

Explicitly out of scope

Extension migrations (E13) and disable/re-enable safety (E14).
Extension settings persistence (E12).

Test plan

Activate an extension, restart, and assert persisted state is unchanged.

Rollback note

Drop the extension-state rows or roll back the schema migration.
