Parent epic: [E11] Extension API v1 (#39)

Intent

Persist per-extension runtime state — installed version, enabled state, config schema version, error diagnostics, and timestamps — so it survives restarts and is queryable in admin.

Acceptance criteria

Stores the installed extension version.
Stores the extension's enabled or disabled state.
Stores the config schema version for the extension.
Stores error diagnostics and timestamps for failed or incompatible extensions.
Persisted state survives process restart and is visible in admin.

Explicitly out of scope

The manifest contract (E11-S01), extension context (E11-S02), and compatibility validator (E11-S03).
Extension settings persistence (E12).
Extension migrations (E13) and disable/re-enable safety (E14).

Test plan

Activate an extension, restart the process, and assert the persisted version, state, and timestamps are unchanged.

Rollback note

Drop the extension-state rows or roll back the schema migration; no content data is affected.
